Worker's Comp Procedure and Forms
Please follow this detailed procedure if you are injured while at work.
Initial reports and treatment:
- Employees must report any work-place injury or illness to his/her direct supervisor.
- When medical care is needed the supervisor shall follow the instructions stated in the First Report of Injury Instructions packet.
- Within 24 hours of the reported injury, the supervisor must complete the “South Dakota First Report of Injury” form with the employee (included in the Worker's Comp Packet download linked below).
- The supervisor completes the “Supervisor’s Report of Accident” form (included in the Worker's Comp Packet download linked below).
- The supervisor shall provide all completed forms to the Human Resources Department and appropriate administration staff.
State law requires employees to give written notice of a work-related injury to their employer within three business days. Employees who fail to provide timely notice risk the loss of their workers’ compensation benefits.
